UNITE COURSE ADDITION/CANCELLATION FORM
Use this form ONLY if you have already completed
the regular online Registration Form to enroll in courses
offered by UNITE. This form is used ONLY to modify enrollment
for the semester for which you have already completed the
regular, longer form.
Complete this form and click the "Submit" button
at the bottom of the page.
Please fill out the entire form. Required
items on this form are marked (*); they will be used for
identification, to determine admission, and to reference your
University academic record.

receives your request via email and then processes it
